放疗对宫颈癌HSP70和bcl-2蛋白表达的影响及意义

摘    要:目的 :探讨宫颈癌组织中热休克蛋白 (heatshockprotein ,HSP) 70和bcl 2的表达与辐射治疗及其预后的关系。方法 :采用免疫组织化学法宫颈浸润性鳞癌47例标本检测放疗前、后 (DT812Gy ,DT2Gy/次、5次 /周 )HSP70、bcl 2的不同表达 ,并定期进行随访。结果 :HSP70在宫颈癌细胞中过表达 ,其表达与病理分级正相关 ,P <0 0 1;放疗后其表达受抑制 ,P <0 0 1,与病理分级呈负相关 ,P <0 0 1,与肿瘤消退正相关 ,P <0 0 1,与远期生存期负相关 ,P <0 0 5。bcl 2蛋白表达与病理分级负相关 ,P <0 0 5 ;放疗后其表达显著下降 ,P <0 0 1,不表达组放疗后肿瘤消退显著 ,P <0 0 1,其远期生存率显著低于阳性表达组 ,P <0 0 5。结论 :测定HSP70表达水平可以作为宫颈癌细胞的增殖、分化及放射敏感性及近、远期疗效的指标 ;bcl 2蛋白表达可作为放射敏感性及近、远期疗效判定的指标

Abstract:OBJECTIVE:To study HSP70 and bcl-2 protein expressions of cervical cancer.METHODS:With immunohistochemical techniques, heat shock protein 70KD (HSP70) and bcl-2 protein immunoreactivity were measured respectively in situ for 47 cases of invasive squamous cell carcinoma (ISCC) before and after administration of a total irradiation doses ranging from 8 Gy to 12 Gy (2 Gy/f,5 f/w).RESULTS:After administration of irradiation HSP70 and bcl-2 protein expressions were significantly lower than those pre-irradiation,P<0.01; strong HSP70 immunostaining and negative bcl-2 immunostaining had good therapeutic effects.1-year and 3-year disease free survivals (DFS)were associated with decreasing HSP70,P<0.05 and increasing bcl-2 immunostaining,P<0.05. Before administration irradiation, HSP70 immunostaining was correlated with increasing histological grade,P<0.01,but bcl-2 immunostaining was inversely,P<0.05.CONCLUSIONS:HSP70 may play an important role in tumor cell proliferation, differentiation, and is related to tumor cell radiosensitiveness;HSP70 and bcl-2 protein expressions may be taken as the makers of therapeutic effects.Positive HSP70 expression and negative bcl-2 expression have favorable responder to radiotherapy.
