156 例后循环梗死患者的预后分析

目的 评估后循环梗死患者的预后并分析其相关因素。方法 回顾性分析2004～2006年在我院神经内科住院且发病时间＜7 d的急性后循环梗死并且随访资料完整的病例156例。所有患者在入院时均进行头颅MRI检查，证实在后循环供血区存在缺血病灶。在病后2年内（平均随访时间13.4±5.0月）对患者进行残障量表（mRS）、Barthel指数（BI）评估，了解患者的预后情况。评价危险因素、病变部位和预后的关系。结果 156例后循环梗死患者中，病后２年内无残障及轻度残障（mRS 0～3分）者占76.3%，严重残障（mRS 4～5分）者占19.9%，死亡者占3.8%。mRS和BI两种评估方法一致性较好（P＜0.01）。BI评估的多因素回归分析显示年龄（OR 1.079，95%CI 1.034～1.125，P <0.01）、多病灶（OR 3.463，95%CI 1.294～9.267，P＝0.013）、卒中史（OR 2.505，95%CI 1.103～5.685，P＝0.025）与病后２年的预后相关。结论 后循环梗死患者的总体预后良好，死亡率低。年龄小、单病灶、既往无卒中史者预后较好。
