原发性肾盂非移行细胞肿瘤八例报告

目的:总结原发性肾盂非移行细胞肿瘤的诊断和治疗体会。方法:回顾性分析我院经病理证实的原发性肾盂非移行细胞肿瘤8例的临床病理特征。结果:本组8例血尿和腰痛是主要临床表现,均行手术治疗,术后病理证实鳞状细胞癌7例,腺癌1例。T3期5例,T4期3例。6例获得随访病人中5例死于全身转移,1例随访至今无瘤生存。结论:原发性肾盂非移行细胞肿瘤临床罕见,诊断较困难,大多数病人疗效较差,可能与就诊时肿瘤已是晚期或对全身治疗反应较差有关。

The Diagnosis and Treatment of Patients with Primary Renal Pelvis Tumors of Nonurothelial Origin(A Report of 8 Cases)

Objective:To review the experience in diagnosis and treatment of patients with primary renal pelvis tumors of nonurothelial origin.Methods:We reviewed the records at our hospital from 1990 to 2006 to identify 8 patients with primary nonurothelial tumors of the renal pelvis.We reviewed the patients' records to collect data on tumor subtype,treatment,recurrence,and survival.Results:Eight patients were identified;7 had squamous cell carcinoma and 1 had adenocarcinoma.All patients had been treated with surgical operation.Five of the tumors were pathologic Stage T3 and 3 Stage T4.Of the 6 patients whose follow-up was available,5 died of systemic metasis,1 was alive during last follow-up without disease.Conclusion:Primary nonurothelial carcinomas of the renal pelvis are rare.Our analysis suggests a poor treatment effect for most patients with these pathologic types,probably resulting from the advanced stage at diagnosis and poor responses to systemic therapy.
