重型颅脑损伤术后大面积脑梗死22例临床分析

目的:探讨分析重型颅脑损伤术后大面积脑梗死的原因及防治方法。方法:分析22例经头颅CT检查证实为重型颅脑损伤术后大面积脑梗死的临床表现、原因及治疗方法。结果:按GOS判断预后的标准,恢复良好5例,中残3例,重残6例,植物生存3例,死亡5例。结论:诸多因素导致重型颅脑损伤术后大面积脑梗死,早期预防、早期开颅减压才能减少大面积脑梗死发生率。切除梗死灶有利于减少死亡率及重残率。

Clinical Analysis on 22 Cases with A Large of Brain Infarction after Operation of Severe Cranial Brain Injury

Objective: To explore the cause and prevent means in patients with a large area of brain infraction after operation of severe cranial brain injury.Methods: The clinical manifestation,cause and treatment in 22 cases with brain infarction after operation of severe cranial brain injure and diagnosed by CT were analyzed.Results:According to GOS(Glasgow Outcome Scale)scores,there were 5 cases with good recovery,3 with moderate neurological deficit,6 with severe neurological dificit,3 with vegetative,and 5 died.Conclusion:The rate of mortality and mutilation are very high and the rescuing is difficult in patients with large area of brain infarction after operation of severe cranial brain injury.Many factors may lead to this situation.Early prevention and craniotomy may reduce the incidence of large area of brain infraction.To resect infarct focus are benefit to decrease mortality and severe disable rate.
