同期双侧全膝关节置换与选择性单侧膝关节置换术后早期功能康复临床观察

目的：评价同期双侧全膝关节置换与选择性单侧膝关节置换术后早期功能康复情况的差异。方法：将符合条件的60例患者按照置换方案进行分组，双膝组（n=26，52膝）行同期双侧全膝关节置换，单膝组（n=34，34膝）行单侧全膝关节置换。分别对两组患者置换前后的关节活动度、屈曲挛缩度、疼痛评分及HSS评分进行比较分析，并记录并发症的发生率。结果：置换术后1年随访，置换后关节活动度比较差异无统计学意义（P=0．146）；置换后屈曲挛缩度、疼痛评分，双膝组显著低于单膝组（P=0．039，0．04）；同时双膝组置换后HSS评分优于单膝组（P=0．023）。结论：同期双侧全膝关节置换患者在屈曲挛缩度、疼痛症状评分及HSS评分方面优于单侧全膝关节置换术，早期功能效果更好。

The Clinical Observation of Early Functional Recovery Following the Simultaneous Bilateral Total Knee Replacement and Selective Unilateral Knee Replacement

Objective： To evaluate the early functional recovery differences of patients with the simultaneous bilateral total knee replacement or selective unilateral knee replacement.Method： 60 patients in accordance with the replacement were divided into the bilateral total knee replacement group （ n=26, 52 knees ）, and unilateral total knee replacement group （ n=34, 34 knees ） .Respectively, Preoperative and postoperative range of motion （ROM）, flexion contracture, muscle strength, pain score, and HSS ratings were compared, and the incidence of complications were record.Result： There were no significances between two groups in ROM and muscle strength after 1 year follow-up （ P=0.146 ） ： Postoperative flexion contraeture, pain score, discrepancy of the bilateral total knee replacement group were obviously lower than those of the unilateral total knee replacement group （ P=0.039, 0.04 ） ： the postoperative HSS score of the bilateral total knee replacement group was superior to the unilateral total knee replacement group （ P=0.023 ） . Conclusion： The simultaneous bilateral total knee replacement patients in terms of degrees of flexion contracture, pain score and HSS score are better than unilateral total knee arthroplasty, better early function.
