肾移植供受者HLA快速配型软件开发与研究

目的采用计算机程序和数据库技术,开发一个用于肾移植手术的实用软件,以能快速准确地从等待肾移植的、已作过HLA分型的病人列表中查找与供者符合一定HLA配型准则的病人,供外科医师临床决策参考.同时兼有病案管理、术前检查、术中情况和术后随访记录的操作界面,以及多种常用统计报表功能,以满足临床、科研和病人监护的需求.方法 HLA分型数据是基于血清学法检测的抗原特异性;供、受者HLA配型主要考虑HLA-A,B和DR共6个抗原数据;软件开发采用Delphi 5.0.供、受者HLA配型程序模块采用数据库表保存中间结果,以提高查找速度;成活率统计采用Kaplan-Meyer方法.结果和结论经广州珠江医院一年多来的使用结果,证明该软件设计科学合理,操作简便,功能强大,运行稳定,实用性强.对各医院泌尿外科、HLA分型室、以及其他需要进行HLA配型的部门有推广应用价值.

The Development and Study of a Fast HLA Matching Software Used for Rental Transplant

Purpose A fast HLA matching software to find the most suitable individual quickly in need of kidney transplant from the waiting list for an available donor has been developed for surgeon to make clinic decision.The software also includes other functional modules to meet the needs of a urological department in respect of clinic,research and care for patients.Methods HLA data used in the software were based on antigen specificity typed with serological method.A total of 6 antigen data of HLA A, B,and DR locus were considered for HLA matching.The software was develped using Delphi 5 0.A relational database was designed for storing donors'and recipients'HLA data,and also to store the intermediate results to speed up searching.Kaplan Meyer method was used to calculate graft survival rate statistics.Results After more than one year practical evaluation by Zhujiang Hospital,it was proved that this software is easy to install,simply to operate,the design is scientific and reasonable and the most important aspect is:it has stable and powerful function,and it could be used in practical applications with robust performance.It is a powerful tool for hospital urological department and other departments which need HLA matching.
