| 摘 要： | 目的 探讨在严重多发伤中十二指肠破裂的诊治。方法 回顾分析严重多发伤中十二指肠破裂２６例的临床资料。结果 术前及术中确诊２１例,误诊和漏诊５例。行破裂的十二指肠一期修补６例,胃切除,胃肠吻合９例,修补加空肠造瘘４例,修补加十二指肠憩室化３例,十二指肠空肠Ｒｏｕｘ－ｅｎ－ｙ吻合４例。本组治愈１９例（７３．１％）,死亡７例（２６．９％）,结论 正确处理和恰当的术式是救治严重多发伤十二指肠破裂的关键。

| Abstract: | Objective To study the diagnosis and treatment of duodenal rupture in severe multiple injuries. Methods The clinical data of 26 patients with duodenal rupture in severe multiple injuries were retrospectively analysed. Results Of the 26 cases, 21 patients were diagnosed and 5 patients maldiagnosed preoperatively. Of them, 6 cases had one stage repair of ruptured duodenum with simple suture, 9 had gastrectomy with gastroenterostomy, 4 had repair plus jejunostomy, 3 had repair plus duodenumcele, and 4 cases had Roux en Y duodenojejunostomy. Seven patients(26.9%) died while nineteen(73.9%) were cured in this series. Conclusions Correct treatment and appropriate operation are essential for duodenal rupture in severe multiple injuries. |
