摘    要:目的 探讨16层螺旋CT阴性法胰胆管造影(N-CTCP)对壶腹周围癌的诊断价值.方法 回顾分析临床证实的15例壶腹周围癌16层螺旋CT阴性法胰胆管造影胰胆管扩张及其远端的狭窄情况,并观察壶腹周围肿块双期增强的强化程度.结果 15例壶腹周围癌均见胆总管扩张,远段狭窄,其中狭窄呈截断型(扩张胆管在梗阻部10mm内突然消失)8例;漏斗型(扩张胆管在梗阻部呈渐进性狭窄,狭窄段超过lOmm)5例;突然狭窄型(扩张胆管在梗阻部突然狭窄,狭窄段不超过20ram)2例,狭窄部均见肿块或增厚管壁明显强化.结论 16层螺旋CT N-CTCP与增强扫描相结合对壶腹周围癌的诊断有重要价值.

Abstract:Obiective To evaluate the diagnosis value of 16 row spiral CT N-CTCP (negative CT cholangiopancreatography)for periampullary carcinoma. Methods Retrospectively analyzed the 16 row spiral CT N-CTCP images of 15 cases who had periampullary careincma proved by clinic.Learned how much the pancreaticobiliary duct dilated and how much the low part of the duct narrowed.And also learned how much the mass around the raters ampulla enhanced at the double-phase scans. Rusults The common bile duct (CBD)all dilated of this 15 cases, and the low position of the CBD were all narrow. Of this, 8 eases were truncated shape(means the dilated CBD dispears within 10mm from the obstruction position). There were 5 funnel shape cases(the dilated CBD became more and more narrow gradually at the obstnwtion position,and the narrow part were more than 10 mm). And 2 others were abrupt rupture shape(the dilated CBD dispears abruptly at the obstruction position,and the narrow part wasn't beyond 20 mm). The mass or the wall of the CBD which became thick were all enhanced. Conclusion The 16 row spiral CT N-CTCP eombained with enhancement scans has great value for the diagnosis of periampullary carcinema.
