脑梗塞患者并发牙周炎牙龈组织肥大细胞的分布、组织化学与超微结构的研究

摘    要:目的探讨罹患牙周炎的脑梗塞患者牙龈组织中的肥大细胞在牙周炎病理学中所起的作用。方法收集临床上脑梗塞患者并发牙周炎的病人39例,切取正常人牙龈组织10例为对照。采用光镜处理,临界电解质浓度(CEC)测定,电镜标本制备,肥大细胞计数4种方法。结果HE染色后表面扁平上皮组织变薄,在甲苯胺蓝染色标本中肥大细胞呈蓝色,用Alcian蓝-藏红染色后,对照组牙龈肥大细胞有的为蓝色,有的为红色,而脑梗塞并发牙周炎患者主要为蓝色,仅有少数为红蓝混合色。脑梗塞牙周炎组肥大细胞的CEC值也较低;电镜观察,脑梗塞并发牙周炎组织肥大细胞有显著脱颗粒现象,其附近还可见活化的淋巴细胞、浆细胞及成纤维细胞。结论脑梗塞并发牙周炎患者组织中的肥大细胞增多的同时,出现表型的变化,说明肥大细胞可能在牙周炎的病例中起重要作用,脑梗塞患者由于口腔卫生差,比一般牙周炎患者炎症程度重,肥大细胞反应有所不同。

Abstract:Objective This paper explore the pathological function of the mast cells in paradentitist gingival gum of cerebral infarction. Methods We collect 39 cerebral infarction patients who have paradentitist, meanwhile we gather 10 normal gingival gums as contrast group. Then we observe the groups in 4 ways. Which are light microscope, electronic microscope, measure of boundery of CEC, and mast cells counts. Results Flat squamous epithelium tissue get thin in gum tissue after HE dye. Stained by Alcian blue-red dye, the mast cells in contrast group become red or bule dye, only a few are in a mixed colour of red and blue. The amount of CEC of mast cells in paradentitist ginglval gum of cerebral infarction patients was also lower. Obvious degranulation of mast cell in paradentitist gingival gum can be found with electronic microscope. There were activated lymphocytes, plasma cells and fibroblasts nearby. Conclusion All the above indicate that the phenotype of the mast cells changed as the number of mast cells in parakentitist gingival gum of cerebral in farction increased, showing that the mast cells may play an important role in the pathology of paradentitist. Because the health condition of cerebral infarction patient's mouth cavity is poorer than average patient's and the inflammation of paradentitist of cerebral infarction patient is more serious, the reaction of the mast cells is different.
