张力带治疗膑骨骨折的并发症与改进措施

自１９８７年７月－１９９２年１月使用张力带技术治疗膑骨骨折２０例，共发生并发症１７次，占８５％、其中克氏针尖端皮下痛性滑囊炎１１例，占全部并发症的６５％。敬而远之发症包括；克氏针穿破皮肤，钢丝断裂，钢丝“脱套”、克氏针上下滑动等。大多并发症与克氏针远端不弯曲有直接关系。

Complications of patellar fracture treated with tension-Band wiring and its modified measures

patients treated by Tension-Band wiring for patellar fracture from July 1987 to Jan l992.17 times of complications(85%)took place.There were 11 cases of subcutaneous bursitis with pain on the tip of the Kirschner pins(65%).Other complications include:Kirschner pins stab bing the skin, wire broken, wire escaping form the Kirschner pins, Kirschner pins sliding superioin feriorly and so on.Most of the complications had a direct connection with no incurvation of the dis tal portion of the Kirschner pins.16 patients with patellar fracture have been treated with modified technique which both proximal and distal portion of the Kirschner pins had been bent since Feb 1992, only 2 cases(12. 5%) were complicated by mild bursitis.Performing the modified technique must pay attention to the following measures:(1)Bending the Kirschner pins must near the edge of the patellar (2)The Kirschner pins shouldn' t be bent in the same plane.(3) The Kirschner pins and wires should be shosen with sofe materials and with 2mm, 1mm in diameter respectively.
