胰腺癌组织中p53蛋白和IGF-r的表达及临床意义

摘    要:目的:探讨胰腺癌组织中p53蛋白、胰岛素生长因子受体(IGF-r)的表达及临床意义。方法:应用抗生蛋白链菌素-生物素标记法(LSAB)检测39例胰腺癌和19例慢性胰腺炎组织中p53蛋白和IGF-r的表达,并对胰腺癌和慢性胰腺炎及不同分化程度胰腺癌组织中p53蛋白和IGF-r的阳性表达率进行比较。结果:(1)p53蛋白、IGF-r的表达产物主要定位于细胞核或细胞浆中,细胞分布不均匀;(2)胰腺癌组织中p53蛋白、IGF-r的阳性表达率分别为61.5%、66.7%,明显高于慢性胰腺炎组织(5.2%、36.8%)(P〈0.05);(3)高分化胰腺癌中p53蛋白、IGF-r的阳性表达率分别为20.0%、33.3%,明显低于中、低分化胰腺癌(81.8%~92.3%、76.9%~81.8%)(P〈0.05)。结论:(1)p53抑癌基因突变与IGF-r的异常表达在胰腺癌的发生、发展过程中具有重要作用;(2)p53蛋白、IGF-r的过度表达与癌组织的分化程度有关;(3)p53蛋白与IGF-r可以作为鉴别胰腺良、恶性病变和进行诊断及判断预后的分子指标。

Abstract:Objective: The alterative expression of p53 protein and IGF-r (insulin-like growth factor receptor) in pancreatic carcinoma tissues may offer some important information concerning the etio-pathogenesis of this disease and accumulate objective data of molecular pathology for diagnosis and therapy practice. Methods: Immunohistochemical detection of p53 protein, IGF-r in 39 specimens of pancreatic carcinoma and 19 specimens with chronic pancreatitis, and the positive frequency of these tissues were analyzed. Re-sults: The positive rates of p53 protein and IGF-r immunoreactivity were 61. 5%(24/39) and 66. 7%(26/ 39) in tumor tissues, they were significantly higher than that in chronic pancreatitis (5. 2% and 36. 8%) (P <0. 05) ; In cancer tissues, the positive frequency of p53 and IGF-r was 20. 0% (3/15) and 33. 3% (5/ 15) in well differentiated carcinoma, which showed significant lower than in moderately and poorly differentiated one (P <0. 05). Conclusions: (1) p53 mutation and IGF-r overexpression may have critical effect on pancreatic carcinoma; (2) Overexpression of p53 and IGF-r is associated with pathohistologic grading of pancreatic carcinoma; (3) Detection of these biomolecle may offer the useful index for diagnosis and therapy of pancreatic benign and malignant lesions.
