腹腔镜切除或开窗治疗单纯性肝囊肿

目的探讨经腹腔镜治疗单纯性肝囊肿的手术方式及效果。方法回顾分析2002-06～2008-10经电视腹腔镜治疗巨大(>10cm)肝囊肿(切除与开窗术)116例患者的临床资料。结果116例手术皆获成功,其中,7例为交通性,确定交通孔后予以缝合+网膜固定,无中转开腹;同时施行胆囊切除19例、肝缘血管瘤切除6例、肠粘连松解手术15例。手术时间11～59min(平均26min);术后3～8d出院。术后1个月B超检查:囊肿开窗后消失89例(76.7%),囊肿<3cm者21例(18.1%),囊肿3～5cm者6例(5.2%),无>5cm者。术后3个月B超复查:囊肿<3cm者16例(13.8%),囊肿3～5cm者3例(2.6%),仍无>5cm者。无胆瘘、出血等手术并发症发生;术后1年B超复查59例囊肿无复发。结论采用微创的手术方式治疗单纯性肝囊肿,创伤小、操作简便、手术视野清晰、疗效肯定、恢复快、住院时间短、费用低。

Laparoscopic excision or alone windowing in the treating simple cyst of the liver in 116 cases

Objective To explore the method of operation for treating simple cyst of the liver.Method Clinical data of 116 cases with simple cyst of the liver were treated by laparoscopic excision or alone windowing from June 2002 to October 2008 in author's hospital were retrospectively studied.Result All the cases were succeed by above two procedures.All cyst of the liver had disappeared off by ultrasound one month after treatment.All the cases did not relapse by ultrasound 3 months after treating.The 116 cases were cured.Conclusion According to the results of laparoscopic excision or alone windowing it is showed that above operations for treating simple cyst of the liver has such as less trauma,simpler manipulation,clear surgery field,better effects,faster reeovery,shorter hospitalazation duration and less cost etc.
