老年2型糖尿病患者尿Ⅳ型胶原测定的意义

目的 探讨老年人群及老年2型糖尿病患者尿Ⅳ型胶原测定的变化意义。方法 随机选取96例老年2型糖尿病患者、90例非老年2型糖尿病患者（尿微量白蛋白阴性）和93例老年非糖尿病人，采用ELISA法测定尿Ⅳ型胶原，分析其在老年糖尿病患者中的意义。结果 （1）老年糖尿病组尿Ⅳ型胶原（13．37±6．42）ng／ml]明显高于老年非糖尿病患者（10．52±6．44）ng／ml]（P〈0．05）。（2）老年糖尿病组尿Ⅳ型胶原与非老年糖尿病组尿Ⅳ型胶原（10．41±7．53）ng／ml]有显著差异（P〈0．05），2组间糖尿病病程无明显差异。结论 （1）老年糖尿病患者相对于非老年糖尿病，以及老年非糖尿病患者尿Ⅳ型胶原显著性增高。（2）老年2型糖尿病患者尿Ⅳ型胶原的升高早于尿微量白蛋白，检测尿Ⅳ型胶原有助于早期发现老年糖尿病患者肾损害。

Significance of detection of urinary type IV collagen in elderly patients with type 2 diabetes mellitus

Objective To investigate the significance of urinary type Ⅳ collagen in elderly patients with type 2 diabetes. Methods Ninety-six elderly patients with type 2 diabetes mellitus （T2DM） ,90 middle-aged patients with T2DM, and 93 elderly people without T2DM were randomly enrolled. Urinary type Ⅳ collagen was measured by the enzyme linked immunosorbent assay （ELISA）, and the results were compared among the groups. Results （ 1 ） Urinary type Ⅳ collagen was much higher in elderly patients with T2DM than that in elderly people without diabetes （ P 〈 0. 05 ）. （2） Urinary type Ⅳ collagen increased significantly in elderly patients with T2DM than that in middle-aged patients with T2DM（ P 〈0. 05）. Conclusions （ 1 ） Urinary type Ⅳ collagen was much higher in elderly patients with T2DM. （2） Urinary type Ⅳ collagen is useful in diagnosis of renal lesions in elderly patients with type 2 diabetes because it appears before microalbuminaria.
