摘    要:目的评价无创正压通气(NIPPV)早期介入治疗急性左心衰的临床效果。方法对28例符合早期介入指标的急性左心衰患者,在常规抗心衰基础治疗上加用NIPPV治疗,观察其临床效果、并发症及不良反应。结果 28例急性左心衰患者应用NIPPV治疗后,无一例患者需要气管插管机械通气,临床症状明显改善;NIPPV治疗前后患者的生命体征及生化结果,有明显差异(P<0.01);对比过去3年30例符合早期介入指标给予常规治疗的急性左心衰竭患者,有效率66.67%,需有创机械通气10例,有创通气率33.3%,两组有效率及有创通气率有明显差异(均P<0.05)。结论 NIPPV早期介入治疗急性左心衰效果显著,安全性高,并发症少,适宜在临床推广应用。

Abstract:Objective To evaluate the clinical efficacy of noninvasive positive pressure ventilation (NIPPV) as an early interventional treatment of acute left vantricular failure (ALVF). Methods We assigned 28 ALVF patients with the indications of early intervention to the conventional basic anti-heart failure therapy plus NIPPV therapy. The primary outcome was a composite of clinical efficacy, complications and adverse reactions. Results None of the 28 patients required intubation and mechanical ventilation, and their clinical symptoms improved. The patients&#39; pre-NIPPV and post-NIPPV vital signs and biochemical results were significantly different (P 〈0.01);Comapred to the efficiency of 66.67%in the 30 patients with indications of early intervention but received conventional treatment in the past 3 years(10 patients required invasive mechanical ventilation), the efficiency of the two groups and the rates of invasive ventilation were significantly different (P〈0.05). Conclusions Early intervention of NIPPV is an effective therapy for acute left ventricular failure, which has significant curative effect, higher safety, fewer complications, and is suitable for clinical application.
