难治性支原体肺炎的肺外表现及免疫因子观察

目的探讨难治性支原体肺炎的肺外表现和免疫学发病机制。方法回顾性分析2011年2月至2013年3月期间我院收治的25例难治性支原体肺炎患儿资料,设为观察组,同时选择来我院正常体检的健康儿童20例作为对照组。两组均进行血清实验室检查以及胸部的X线片检查和CT扫查,比较两组的免疫因子指标(IgG含量、IgM含量和CD3+/CD8+、CD3+、CD3+/CD4+、CD4+/CD8+和CD3+/CD25+),并统计X线胸片检测和CT检测患儿的病灶类型。结果观察组患儿IgG含量[(7.6±1.8)g/L],IgM含量[(1.7±0.2)g/L]和CD3+/CD8+[(31.6±2.2)%]指标均明显高于对照组(P<0.05)。25例难治性支原体肺炎患儿肺部的病灶情况均具有比较明显的影像学特点。结论难治性肺炎支原体疾病的发生与体液免疫过程紧密相关,影像学检测方法对尽早发现难治性支原体肺炎病灶具有重要意义。

Observation of Extrapulmonary Manifestations and Immunological Factors of Refractory Mycoplasma Pneumonia

Objective To discuss the extrapulmonary manifestations and immunological factors of refractory mycoplasma pneumonia. Methods 25 children with refractory mycoplasma pneumonia in our hospital from February 2011 to March 2013 were selected as the observation group, and 20 cases of healthy children were selected as the control group. Both groups received serum laboratory tests and imaging examination of chest including X-ray and CT scan. The indicators of immune factors （IgG content, IgM content, CD3＋/CD8＋, CD3 ＋, CD3＋/CD4＋, CD4＋/CD8＋ and CD3＋/CD25＋） were compared between two groups, and the lesion types of children with refractory mycoplasma pneumonia detected by X-ray and CT were analyzed. Results The contents of IgG and IgM, and CD3＋/CD8＋were （7.6±1.8） g/L, （1.7±0.2） g/L, and （31.6±2.2）%respectively, significantly higher than those of control group （P〈0.05）. The lung lesions of all 25 children with refractory mycoplasma pneumonia had significant imaging characteristics. Conclusions The incidence of refractory mycoplasma pneumonia has close relationship to humoral immune process. Imaging detection has important significance for early detection of lesions of refractory mycoplasma pneumonia.
