糖尿病合并甲状腺功能亢进症的发病机制及临床诊治探讨

目的 探析糖尿病病甲状腺功能亢进症的临床疗法、发病因素及临床效果.方法 入选我院糖尿病合并甲亢老年患者102例,根据甲状腺功能检查、监测血糖、结合甲亢及糖尿病的体征、症状进行确诊,后实施抗甲亢、降糖及对症疗法,对该病的临床疗法、发病因素及临床效果等资料进行回顾性分析.结果 治疗后患者的睡前血糖、餐后2h血糖及空腹血糖均明显低于治疗前（P ＜0.05）;治疗后患者的游离甲状腺素（FT4）、游离三碘甲状腺原氨酸（FT3）、总甲状腺激素（TT4）、总三碘甲状腺原氨酸（TT3）等指标变化明显低于治疗前（P ＜0.05）.结论 糖尿病并甲亢的发病与甲状腺激素（TH）通过儿茶酚胺异常血糖调节、异常胰岛素分泌等因素密切相关,综合治疗、尽早明确诊断可提升临床效果,提升病人的生活质量.

Discussion on Pathogenesis,Clinical Diagnosis and Treatment of Diabetes Combined with Hyperthyroidism

Objective To discuss and analyze the clinical treatment, pathogenic factors and clinical effect of diabetes combined with hyperthyroidism. Methods 102 elderly patients with diabetes combined with hyperthyroidism were diagnosed according to thyroid function test, blood sugar monitoring, signs and symptoms, and then were given with anti-hyperthyroidism, blood glucose reducing and symptomatic treatment. The data of clinical treatment, pathogenic factors and clinical effect were analyzed retrospectively. Results After treatment, blood sugar before sleeping, 2 hour postprandial blood glucose and FBG in patients were obviously lower than those before treatment （P 〈0.05）; indicator changes of FT4, FT3, TT4 and TT3 were lower than those before treatment （P 〈0.05）. Conclusions Onset of diabetes combined with hyperthyroidism has close relation with factors of abnormal blood glucose regulation of TH through catecholamine and abnormal insulin secretion. Comprehensive treatment and early definite diagnosis can improve the clinical effect and the life equality of patients.
