阻塞性睡眠呼吸暂停低通气综合征对右心室结构功能改变的影响分析

摘    要:目的 研究探讨阻塞性睡眠呼吸暂停低通气综合征(obstructive sleep apnea hypopnea syndrome,OSAHS)患者右心室结构功能的改变情况,从而为相关的临床研究提供借鉴和参考.方法 选取我院2010年12月至2012年12月期间收治的阻塞性睡眠呼吸暂停低通气综合征患者44例,同时选取同期我院健康体检者44例进行比较研究.结果 在右室面积变化率、舒张早期峰值速度、舒张晚期峰值速度等指标上,组间比较,指标水平差异不大,且P均>0.05,说明数据差异无统计学意义;在右心室等容收缩期加速度、右心室末期舒张容积、右心室博出量上,组间比较,OSAHS组人群的指标水平显著地低于对照研究组人群的指标水平,且P<0.05,说明差异具有统计学意义.结论 在临床上阻塞性睡眠呼吸暂停低通气综合征患者会出现右心室结构功能改变,主要表现为右心室等容收缩期加速度指标水平显著降低.

Abstract:Objective To analyze the influence of obstructive sleep apnea hypopnea syndrome (OSAHS) on changes of right ventricular structure and function, and provide a reference for the relevant clinical studies. Methods Forty-four cases of patients with OSAHS from December 2010 to December 2013 were selected in this study, then 44 cases of healthy volunteers were chosen as contrast. Results There was no significant difference in right ventricular fractional area change, early diastolic peak velocity and late diastolic peak velocity between two groups, P 〉0.05. Right ventrieular acceleration during isovolumic contraction phase, right ventricular end diastolic volume, and right ventficular degression volume of OSAHS group was significantly lower than the healthy volunteers group, P 〈0.05. Conclusions The patients with OSAHS have changes in right ventricular structure and function, mainly manifesting as significant decrease of the right ventricle myocardial acceleration indicators during isovolumic contraction.
