氟烷、氯胺酮与利多卡因在小儿麻醉中的应用

目的观察氟烷、氯胺酮与利多卡因在小儿麻醉的疗效及安全性。方法将130例手术患儿随机分为观察组和对照组各65例,观察组给予氟烷、氯胺酮及利多卡因麻醉,对照组行氟烷和氯胺酮麻醉,比较两组麻醉效果、术中心率（HR）、呼吸频率（RR）、血压（BP）变化及不良反应。结果两组患儿均获得满意的麻醉效果;术中观察组HR、RR均较对照组减慢（P〈0.01）,BP也较对照组降低（P〈0.01）;术后观察组苏醒及清醒时间均较对照组缩短（P〈0.01）;观察组恶心、呕吐、屏气、肌僵、寒颤及躁动等不良反应均较对照组减少（P〈0.05）。结论氟烷、氯胺酮与利多卡因联合麻醉具有诱导快、术中镇静完全、术后清醒快、不良反应少的优点,具有广阔的应用前景。

Clinical application of combined anesthesia with halothane,ketamine and lidocaine in pediatric anesthesia

Objective To explore the clinical effect and safety of combined anesthesia with halothane,ketamine and lidocaine in pediatric anesthesia.Methods 130 pediatric patients undergoing operation were randomly divided into observation group and control group（n = 65 each） .The patients in observation group received combined anesthesia with halothane,ketamine and lidocaine.The patients in control group received halothane combined with ketamine.The anaesthetic effect,heart rate（HR）,respiratory rate（RR）,blood pressure（BP） and adverse reactions of two groups were compared.Results All patients received satisfactory anaesthetic effect;The HR,RR and BP in observation group were significantly lower than those in control group during operation（all P〈0.01）;The time of palinesthesia and consciousness in observation group were significantly shorter than those in control group after operation（P〈0.01）;The incidence of nausea,vomit,breathholding,myo-rigid,shivering and restlessness,etc.the observation group were significantly lower than those control group（P〈0.05） .Conclusion It is safe and feasible to give combined anesthesia with halothane,ketamine and lidocaine in pediatric operations,with an incidence of adverse reaction reduced.
