摘    要:目的比较两次粪检对提高改良加藤法(Kato—Katz)诊断血吸虫病敏感性的效果。方法在血吸虫病重度流行区采用Kato—Katz法于1周内每个居民收集2份粪样,每份粪样三张涂片,观察比较单次粪检人群血吸虫感染率以及两次粪检人群血吸虫总感染率和感染度。结果单次粪检人群血吸虫感染率分别为11.46%、9.26%,单次粪检之间人群血吸虫感染率无显著差异(x2=2.84,P>0.05),两次粪检人群血吸虫总感染率为14.94%,显著高于第1次(x2=5.78,P<0.05)和第2次粪检感染率(x2-=16.56,P<0.01)。病人感染度EPG低于40者,1片漏检率高达91.08%,2片漏检率为58.41%;EPG在41-100之间,1片漏检率达20.84%,2片漏检率仅为4.17%;EPG为101~200时,1片漏检率为7.69%,2片漏检率为2.56%;当EPG大于200时,漏检率为0。结论增加粪便送检次数,可提高Kato—Katz法诊断血吸虫病的敏感性, 单次粪检低估人群血吸虫实际感染率。

Abstract:Objective To compare the sensitivity for the methods of Kato-Katz to diagnose schistoso-miasis by two single feces samples. Methods To collect two feces samples of the people in serious endemic regions in a period of one week to examine schistosomisisi by the method of Kato-Katz, one sample three smears. To observe the infection rate and infection degree of the people. Results The infection rate was 11.46% and 9.26% respectively between the two single feces examination. There was no remarkably difference between the two single examinations. The total infection rate was 14.94% and remarkably higher than the two single examinations. The rate of one smear was 91.08%, and two smears was 58.41% when EPG was less than 40. The rate of one smear was 20.84%, and two smears was 4.17% when EPG was from 41 to 100. The rate of one smear was 7.69%, and two smears was 2.56% when EPG was from 101 to 200. The rate of was zero was more than 200. Conclusion It would increase the sensitivity for Kato-Katz to diagnose schistosomiasis when increase the number of examination. Single examination would less evaluation the real infection rate of schistosomiasis.
