改良式玻璃体切割术的临床研究

摘    要:目的 改良玻璃体切割术中的结膜和巩膜切口以最大限度减少组织损伤。方法 将176个玻璃体切割术后患者的176眼分为改良切口组(108眼)和传统切口组(68眼)。改良组眼结膜切口位于11:30~12:30,然后沿角巩膜放射状剪开,巩膜灌注穿刺口位于12:00平坦部。传统切口组进行原半周或全周角巩膜结膜切开,巩膜灌注口位于颞下平坦部。对术后1~2个月的资料进行分析。结果 手术后结膜伤口愈合良好,三面镜下未见周边灌注口处牵拉和异常。两组手术疗效无明显差异,未见任何术中和术后并发症。结论 改良玻璃体切割术操作时间短,组织损伤小,因而优于传统切口手术。

Abstract:Objective To change the operative incision during the vitrectomy for reducing ocular injury to a minumum. Methods 176 eyes of 176 patients received vitrectomy were divided into modified incision group (108 eyes) and conventional incision group (68 eyes). Conjunctival cutting along 11 : 30 - 12- 30 was made in modified incision group, and the scleral irrigation site was at 12:00. A half of circle or a whole circle of conjunctival cutting was made in the conventional incision group, and the scieral irrigation site was at the pans plana of inferior temporal. The data in post-operative 1 -2 months were recorded and analyzed. Results The operative cutting was healed well,and no retinal traction and abnormality were seen at scieral irrigation site in modified incision group. There was not significant difference in operative efficacy between two group,and no any intra- or post-operative complication was found in two groups. Conclusion Modified operation procedure is superior to conventional one because of less injury and shorten operative time.
