单纯性肥胖、2型糖尿病人的血清瘦素水平

目的　探讨单纯性肥胖、2型糖尿病人的血清瘦素 (L P)水平与肥胖度、胰岛素的关系。　方法　受试者精确测量身高、体重、腰围、臀围。禁食 10 h取晨血测血浆血糖 (FPG)、胰岛素 (FINS)、瘦素 (L P)、血脂。应用免疫放射法测定血清 L P水平。　结果　 (1)女性瘦素水平均明显高于男性 ;肥胖组男性、女性瘦素均较正常组、糖尿病组增高 ;糖尿病组与正常组比较瘦素水平无明显差异。 (2 )相关分析显示 ,瘦素与体重指数 (BMI) (P<0 .0 0 1)、性别(P<0 .0 1)、胰岛素 (P<0 .0 5 )相关。较正 BMI、性别后瘦素仍与胰岛素相关 (P<0 .0 5 )。 (3) BMI相近情况下 ,糖尿病组中男性、女性均发现长期使用胰岛素者瘦素、胰岛素均较口服药者高。　结论　机体的体脂量、性别、胰岛素都是调节瘦素的重要因素。高瘦素血症可能与高胰岛素血症、胰岛素抵抗有关

Serum Leptin Level in Type 2 Diabetes and Obesity

Objective\ To investigate the relationship among serum leptin and body fat, gender, age and insulin in obese and type 2 diabetes.\ Methods\ The serum leptin concentrations were measured in normal, obeses or type 2 diabetes by immunoradiometric assay(IRMA).\ Results\ In all three groups, leptin concentrations were higher in obese than other two groups and higher in females than males.\ There were no difference between type 2 diabetes and normal subjucts.\ In a multivariate regression analysis, leptin was associated with gender(P<0 001), body mass index(BMI,P<0 01) and fasting insulin(P<0 05).\ Diabetic subjects treated with insulin had both raised insulin and leptin concentrations with same gender and BMI.\ Conclusion\ Serum leptin concentrations are correlated with BMI, gender, and insulin in normal, obese and type 2 diabetes.\ High leptin concentration may be related to hyperinsulinemia or insulin resistance.
