肺心病合并肾功能不全89例临床分析

目的探讨肺源性心脏病合并肾功能不全的发病机制,临床特点及治疗,以提高该类疾病的诊断治疗效果。方法回顾性分析本院2000～2008年肺源性心脏病合并肾功能不全病例,分析其发病原因,临床表现,肾功能不全程度及预后。结果89例肺源性心脏病并发急慢性肾功能不全患者,与长期使用对肾脏有损害药物和严重缺氧、心力衰竭、上消化道出血、休克等并发症密切相关,药物因素占了55.06%。治疗后显效21例（23.6%）,有效36例（40.45%）,无效10例（11.24%）,死亡20例（22.47%）,其他不明显原因的出院2例（2.25%）。结论肺源性心脏病并发急慢性肾功能不全发病率高,对治疗反应差,预后差。根据肾功能情况选药及早期干预对预后可起一定作用。

89 case clinical analysis of Pulmonary heart disease comloined renal insufficiency

Objective To research pulmonary heart disease and renal insufficiency in the pathogenesis,clinical features and treatment in order to improve the diagnosis and treatment of diseases.Methods Retrospective analysis of 2000~2008 in our hospital pulmonary heart disease and renal insufficiency patients,analyze its causes,clinical manifestations,renal insufficiency and prognosis.Results 89 cases of pulmonary heart disease complicated by acute or chronic renal insufficiency in patients with long-term use of drugs for kidney damage and severe hypoxia,heart failure,upper gastrointestinal bleeding,shock and other complications were closely related factors accounted for 55.06% of drugs.Markedly after treatment in 21 cases（23.6%）,effective in 36 cases（40.45%）,ineffective in 10 cases（11.24%）,died 20 cases（22.47%）,other obvious causes of hospital in 2 cases（2.25%）.Conclusion Acute and chronic pulmonary heart disease complicated by a high incidence of renal dysfunction on the response to treatment is poor,and poor prognosis.Drugs are selected according to renal function and early intervention can play a role in prognosis.
