广州地区肥胖儿童瘦素水平与生长发育关系的研究

目的　研究广州地区单纯性肥胖儿童血清瘦素水平与身高体重的关系。方法　用放射免疫分析法测定儿童血清瘦素水平。结果　 (1)肥胖儿童其血清瘦素水平均与体重指数 (BMI)呈正相关 ;(2 )肥胖儿童的血清瘦素水平在 7～ 11岁逐步上升 ,11～ 13岁则整体下降 ;(3)肥胖儿童瘦素偏高 (P <0 .0 0 1) ,在相同的BMI水平 ,肥胖男孩的血清瘦素浓度低于肥胖女孩 ;(4)肥胖男孩在以瘦素为因变量 ,BMI、年龄、分期为自变量的逐步多元回归统计中 ,BMI是与瘦素关系最为密切的因素 (r =0 .13,P <0 .0 5 ) ;而对于肥胖女孩 ,影响瘦素水平最显著因素则是年龄 (r =0 .378,P <0 .0 0 1) ,BMI次之 (r =0 .36 1,P <0 .0 0 0 1)。结论　 (1)肥胖男孩的血清瘦素水平与BMI密切相关。而肥胖女孩血清瘦素水平与年龄密切相关 ;(2 )与正常儿童不同 ,随着青春期发育 ,肥胖儿童的血清瘦素先上升后下降 ,提示瘦素在肥胖儿童的这一变化规律可能影响肥胖儿童青春期的进一步发育 ,值得继续研究

The relationship between serum leptin concentration and growth in obese children from Guangzhou region

Objective To study the relationship between serum leptin concentration and hight and weight in obese children from Guangzhou region. Methods Serum leptin concentration was measured by RIA in nonobese children and simply obese children. Results (1)Serum leptin level was positively correlated with body mass index (BMI) in all the simple obese children; (2) Leptin concentration in all the simply obese children increased steadily from 7 to 11 years, but decreased from 11 to 13 years; (3)Leptin concentration in simply obese children was significanly higher than that in normal children ( P <0.001 ). Leptin in obese boys was lower than that in obese girls at the same level of BMI. (4) In obese boys, BMI was found to be the most important one among the factors which affected the leptin levels by multiple stepwise regression analysis taking leptin as the dependent variable, and BMI, age, stage as the independent variables ( r = 0.13 , P <0.05 ). But in obese girls, it was age ( r = 0.378 , P <0.001 ), BMI next ( r = 0.361 , P <0.001 ). Conclusions (1)Serum leptin concentration in obese boys strongly correlates with BMI, but in obese girls, it is the age; (2) Different changes of leptin between obese children and normal children during the puberty may affect the further development of obese children.
