金葡球菌和肺炎链球菌在体内对莫西沙星耐药性分析

摘    要:目的:分析金葡球菌和肺炎链球菌在体内对莫西沙星的耐药性。方法:采用大鼠肉芽肿袋模型,体内试验选用野生株、第一代耐药株以及多代耐药株,使用不同菌株感染大鼠产生肉芽肿袋后分为A、B、C3个组,用莫西沙星进行不同剂量用法治疗,每天收集肉芽肿袋的渗出物,根据耐药株的产生测定金葡球菌与肺炎链球菌对莫西沙星的耐药性。结果:由于莫西沙星对试验菌株与大鼠肉芽肿袋中分离菌的MIC相同,所以敏感性无显著变化。两种感染病原体A组与B组的试验结果相同,A组治疗1d后肺炎链球菌减少约107CFU·mL-1,A组治疗1d后金葡球菌的减少数量较慢,3d后减少数量明显。 B组的肺炎链球菌与金葡球菌的生存率都迅速下降,但减少幅度差异不明显,莫西沙星对细菌生存数量的影响可持续6d,影响期间细菌没有发生再生长,莫西沙星对肺炎链球菌的杀菌能力高于对金葡球菌的杀菌能力。同时B组与C组对比,50mg·kg-1·d-1的莫西沙星杀菌速度和程度都会明显降低,但杀菌效果仍在,治疗1d后细菌可降低103CFU·mL-1,效果持续4d,但50mg·kg-1·d-1的莫西沙星、感染后24h治疗对多代耐药株的疗效几乎为零。结论:即使人体内莫西沙星的浓度低于预期的治疗浓度,也对金葡球菌与肺炎链球菌的野生株与第一代耐药株具有良好的杀菌效果。
