五虎汤加减联合阿奇霉素辨治儿童支原体肺炎的疗效及对T细胞亚群水平的影响

摘    要:目的:观察五虎汤加减联合阿奇霉素辨治儿童支原体肺炎的疗效及对T细胞亚群水平的影响。方法:按照随机数字表法将2020年1月至2021年1月在我院就诊的70例儿童支原体肺炎患儿分为对照组和观察组,每组各35例。对照组采用常规西医治疗,观察组在对照组治疗的基础上加用五虎汤治疗。比较两组患儿临床疗效及治疗前后中医证候积分、T细胞亚群水平、炎性因子水平等。结果:对照组有效率为82.86%,观察组有效率为97.14%,两组有效率比较,差异有统计学意义(P<0.05);两组患儿治疗后咳嗽气喘、胸高胁满、发热恶风、咳痰黄稠、鼻翼翕动、面赤心烦、口唇发绀等积分均低于治疗前,差异有统计学意义(P<0.05);组间比较,差异有统计学意义(P<0.05);两组治疗后CD3+、CD4+、CD4+/CD8+水平高于治疗前,CD8+水平低于治疗前,差异有统计学意义(P<0.05);组间比较,差异有统计学意义(P<0.05);两组患者治疗后血清C反应蛋白(C-reactive protein,CRP)、肿瘤坏死因子-α(tumor necrosis factorα,TNF-α)、γ干扰素(interferon-γ,IFN-γ)、白细胞介素-6(interleukin-6,IL-6)、白细胞介素-10(IL-10)水平均低于治疗前,差异有统计学意义(P<0.05);组间比较,差异有统计学意义(P<0.05)。结论:五虎汤加减联合阿奇霉素能够有效改善支原体肺炎患儿T细胞亚群水平和炎性因子水平,改善临床症状。

Abstract:Objective:To observe the clinical curative effect of Modified Five-Tiger Decoction combined with azithromycin on mycoplasma pneumonia in children and its Influence on the level of T cell subsets.Methods:According to the random number table method,a total of 70 children with mycoplasma pneumonia in our hospital from January 2020 to January 2021 were divided into the control group and the observation group,with 35 cases in each group.The control group was treated with conventional Western medicine,while the observation group was treated with Five-Tiger Decoction on the basis of the control group.The clinical efficacy,TCM syndrome score,T cell subsets and inflammatory factors were compared between the two groups before and after treatment.Results:The effective rate was 82.86%in the control group and 97.14%in the observation group.There was significant difference between the two groups(P<0.05).After treatment,the scores of cough and asthma,chest and hypochondriac fullness,fever and aversion to wind,yellow and thick expectoration,moving nose wings,red face andvexation,cyanosis of lips in the two groups were lower than those of the same group before treatment,and all the differences were statistically significant(P<0.05);and all the differences in the above indexes were also statistically significant between two groups.After treatment,the levels of CD3+,CD4+,CD4+/CD8+in the two groups were higher than those of the same group before treatment,and CD8+was lower than that before treatment and all the differences were statistically significant(P<0.05);and all the differences in the above indexes were also statistically significant between two groups.The levels of serum CRP-α,IFN-γ,TNF-γ、IL-6、IL-10 in the two groups after treatment were lower than those of the same group before treatment,and all the differences were statistically significant(P<0.05);and all the differences in the above indexes were also statistically significant between two groups.Conclusion:Five-Tiger Decoction combined with azithromycin can effectively improve the level of T cell subsets and inflammatory factors in children with mycoplasma pneumonia,and enhance clinical symptoms.
