社区糖尿病人血糖控制及影响因素分析

摘    要:目的了解社区糖尿病患者血糖控制水平及影响因素,为社区糖尿病人的管理提供参考。方法抽样调查社区糖尿病患者,了解其基本情况、生活方式,检测患者空腹血糖和糖化血红蛋白。结果218名患者空腹血糖平均值为8.8mmol/L,空腹血糖控制在6.1mmol/L以下的有45人(20.64%);214名患者糖化血红蛋白平均值为8.9%,糖化血红蛋白控制在7.0%以下的有70人(32.71%)。每周运动时间较多的患者空腹血糖、糖化血红蛋白水平低于运动时间较少的患者(P<0.05),控制较好的患者数所占比例较高。随着病程的延长,患者空腹血糖水平的均值从病程≤2a的7.2mmol/L上升到病程≥6a的9.5mmol/L,空腹血糖控制较好的比例从44.44%下降到16.00%;糖化血红蛋白均值从7.8%上升到9.5%,糖化血红蛋白控制较好的比例从53.85%下降到23.47%,差异均有统计学意义。文化程度及饮食类型对患者血糖控制的影响无显著作用。结论应进一步加强社区糖尿病人的管理,控制患者血糖水平。通过适当的糖尿病管理干预即使文化程度较低的患者也能获得预期的效果。对高血糖的治疗应依据病程分阶段进行。

Abstract:Objective To evaluate the blood glucose control and analyze its determinants in order to provide reference for community diabetes management. Methods A questionnaire survey was performed in 218 DM patients selected from communities. Fasting plasma glucose (FPG) and hemoglobin A1c (HbAlc) were tested. Results The mean FPG concentration was 8.8 mmol/L. Among 218 DM patients, 45 (20.64%) patients' FPG were below 6.1 mmol/L and the mean HbAlc concentration was 8.9% . 70 ( 32.71% ) patients' HbAlc were below 7.0% . Patients who spent more time on sports had significantly lower FPG concentrations than those spent less time. FPG concentration in patients with durations of DM shorter than 2 years was 7.2 mmol/L, and the percentages of good control was 44.44% . FPG concentration in patients with durations of DM longer than 6 years was 9.5 mmol/L, and the percentages of good control was 16.00%. HbAlc concentration in patients with durations of DM shorter than 2 years was 7.8%, and the percentages of good control was 53.85%. HbAlc concentration in patients with durations of DM longer than 6 years was 9.5%, and the percentages of good control was 23.47% . Blood glucose levels had no significantly difference between different education levels and food styles. Conclusions The management of DM patients in communities should be strengthen to control blood glacose levels.
