肝血管平滑肌脂肪瘤11例临床病理分析

目的：研究肝血科平滑肌脂肪瘤（HAML）的临床病理特点。方法：收集临床相关资料，采用常规HE切片、特殊染色、免疫组化、电镜等方法对本院8例及会诊3例HAML进行研究。结果：11例HAML病人影像学及症状、体征均有其特征，病理形态学上均显示有血管周上皮样细胞，血管分厚壁及薄壁两种，皮皮样细胞分为4种形态，形态与免疫表型有一定相关关系，PAS部分上皮样细胞浆阳性且抗淀粉酶消化，电镜下可见类似前黑色素不上体的特殊晶体样结构及密体。结论：HAML是一种好发于中青年女性的良性肿瘤，肌动蛋白、HMB45阳性为HAML的特点，HAML因其特殊的光镜及电镜下形态、免疫组化特点及其临床特征而归入血管周上皮样细胞相关肿瘤。

Hepatic angiomyolipoma: a clinical and pathological analysis of 11 cases

Objective To investigate the clinical and pathological characteristics of hepatic angiomyolipoma (HAML). Methods Clinical data were collected and HE sectioning, special staining, immunohistochemistry and electron microscopy were used to study 8 cases of HAML treated in our hospital and 3 consulted cases. Results All the 11 patients with HAML had special features in images, symptoms and signs. Microscopy revealed that there were perivascular epithelioid cells presenting 4 shapes. They showed special features under PAS staining and electron microscope. Conclusion HAML is a special kind of benign tumor and its characteristic is positive reaction to both actin and HMB45. HAML is classified as one of perivascular epithelioid cell-related tumors because of its special morphology under light and electron microscope, immunohistochemical features and clinical characteristics.
