慢性胃病患者临床症状与胃运动功能的关系:胃肌电活动和腔内压力变化的同步分析

目的从胃运动功能的角度探讨胃器质性疾病患者各种临床症状的产生机制,以为临床治疗提供依据。方法采用胃窦内同步记录胃肌电活动和腔内压力变化的方法,研究了４５例慢性胃病（含慢性胃炎和胃溃疡）患者有关临床症状与胃运动功能的关系。结果慢性胃病患者中常见胃肌电节律紊乱（以快速型为主）和肌电－压力脱偶联现象;呕吐和反酸分别与慢波节律紊乱和压力波频率减少有关：暖气、腹鸣均与平均压力波幅值及３种不同形态压力波的平均幅值的增高有关;恶心与平台型波平均幅值的下降有关。结论提示上消化道症状的产生与胃肌电节律异常及胃平滑肌收缩功能均有关系,且不同的症状可能以不同的胃运动功能改变为基础。

Relationship between clinical symptoms and gastric dysmotility in patients with chronic gastropathy: a simultaneous analysis of gastric pressure and myoelectrical activity

Objective The mechanism of symptoms in patients with gastric organic diseases was investigated in view of gastric motility to provide clues for treatment. Method A simultaneous recording and analysis of the changes of infragastric pressure and myoelectrical activity were performed in 45 patients with chronic gastritis or gastric ulcer and the data was processed statistically. Results it was shown that myoelectrical disorder (especially tachyarrhythmia and tachygastria) and the decouple of myoelectrical activity and pressure often occurred in the patients with chronic gastropathy. Vomiting and regurgitation were respectively related to slow-wave dysrhythmia and lowered frequency of contraction, while both borborygmus and belching might be attributed to the increase in general amplitude of contraction wave and in the amplitude of the 3 types of contraction waves. Conclusion Common symptoms of chronic gastric diseases relate not only to myoelectrical dysrhythmias but also to disorders of gastric motility.
