| 摘 要： | 目的探讨慢性肝病合并自发性细菌性腹膜炎(spontaneous bacterial periton itis,SBP)早期临床特点,以提高诊治水平。方法回顾性分析我院2008年10月~2009年12月收治的慢性肝病合并SBP早期误诊20例的临床资料。结果本组早期误诊为急性胃肠炎7例,胆道感染4例,上呼吸道感染、门静脉血栓、腹腔肿瘤、肝肾综合征各2例,结核性腹膜炎1例,后根据临床表现、医技检查确诊为典型SBP。结论慢性肝病合并SBP早期临床表现不典型,体征欠缺,腹腔积液白细胞数未达到诊断标准,首次误诊以急性胃肠炎、胆道感染最多。提示临床医师在诊治过程中一定要重视临床表现的演变,勿过分依赖医技检查结果,以减少误诊。
