结石性胆囊炎患者胆囊粘膜病变分析

目的　探讨结石性胆囊炎患者胆囊粘膜病变与临床的关系。方法　应用组织化学技术 ,对连续收集的 1 0 0例结石性胆囊炎患者的胆囊标本进行观察。按年龄和结石病史不同进行分组比较。结果　 53例粘膜上皮出现化生 ,1 9例出现不典型增生 ,其中重度不典型增生 2例。化生和不典型增生的发生率与患者的年龄、结石病史呈正相关。年龄大于 50岁、结石病史超过 5年 ,胆囊粘膜上皮化生和不典型增生的发生率明显增高。结论　结石是引起胆囊粘膜上皮化生和不典型增生的主要原因。年龄大于 50岁、结石病史超过 5年是胆囊癌发生的高危因素

Analysis of pathologic changes in mucous membrane of gallbladder in patients with lithic cholecystitis

Objective To investigate the relationship between the pathologic changes in mucous membrane of gallbladder in the patients with lithic cholecystitis and clinical features.Methods The gallbladder samples of 100 cases collected continually were observed by using histochemical method. The comparison was carried out in terms of age and case history.Results Metaplasia appeared in epithelial mucous membrane in 53 cases and atypical hyperplasia in 19. Thereinto, 2 cases were diagnosed as having severe atypical hyperplasia. There was a positive correlation between the incidence of metaplasia, atypical hyperplasia and age, case history of the patients. In the patients aged over 50 with case history of more than 5 years, the incidence of metaplasia and atypical hyperplasia in mucous membrane of gallbladder was obviously increased.Conclusions Lithiasis is the main cause of metaplasia in epithelial mucous membrane and atypical hyperplasia. Age (over 50 years old) and case history (more than 5 years) are the high risk factors for gallbladder cancer.
