糖尿病合并肺结核的CT影像特征分析(附62例分析)

目的分析糖尿病合并肺结核的CT影像学表现,提高对该病影像学的认识,以达到早期诊断,提高治愈率。方法收集2010年2月至2012年2月我院收治的62例糖尿病合并肺结核患者,分析其病灶CT的分布特点及形态特点。结果 62例患者中,病灶位于双肺上叶尖、后段和(或)下叶背段者25例,占40.3%;位于右肺中叶和/或左肺上叶舌段者22例,占35.4%;位于一侧和(或)双肺上中前段、双下肺叶基底段者15例,占24.1%。表现为大片状病灶者38例,占61.2%,斑片状病灶、部分融合者34例,占54.8%,合并空洞者32例,占51.6.0%,常伴有支气管播散灶。结论糖尿病合并肺结核患者具有影像学一定特征,常呈大片状分布于多个肺叶肺段,空洞、支气管播散发生率较高。

Diabetes Complicated with Pulmonary Tuberculosis by CT Image Analysis (Analysis of 62 Cases)

Objective To analyze the radiographic and CT appearances of pumonary tuberculosis complicated with diabetes mellitus and improve the comprehension of its imaging features.Methods 62 in our hospital patients with pulnonary tuberculosis complicated with diabetes mellitus were included from February 2010 to February 2012.The morphological distribution features of the pulnonary tuberculosis lesions were analyzed.The diagnostic accuracy of initial raiological exam ination was obtained.Results In 62 patients the pulnonary tuerculosis 25 patients（40.3%） were found located in apical and posterior segments of bilateral upper lobe or back segments of the lower lobe.The lesions of 22 patients（35.4%） were found in the right middle lobe and（or） the lingual segment of left upper lobe.The lesions of the romaining 12 patients（24.1%） were found unilateral and（or） bilateral anterior segment of the upper lobe and bilateral basal segment of lower lobe.The large consolidation lesions occurred in 38 patients（61.2%） and patchy lesions with partially confluence occurred in 34 patients（54.8%）.In 32 patents（51.6%）,the lesions were complicated with cavitations and frequently with bronchus dissemination.Conclusion The imaging findings of pulnonary tuberculosis complicated with diabetes mellitus is some what characteristic.
