原发性中枢神经系统淋巴瘤3例报告并文献复习

目的:探讨原发性中枢神经系统淋巴瘤(PCNSL)病因、发病机制、诊断及有效治疗手段。方法:回顾总结我科3例PCNSL患者临床表现、影像学特征、病理特征、治疗方案及疗效情况。结果:3例患者均为经手术病理确诊为DLBCL。采用全身系统性化疗联合鞘内化疗加或不加全颅脑放疗。3例症状均明显改善。结论:PCNSL多为B细胞来源的NHL。鞘内化疗联合全身化疗治疗PCNSL效果较好,不良反应轻,提示此种方案可作为PCNSL有效治疗选择之一。其他一些新药及治疗方法取得了一定效果,值得进一步研究。

Primary central nervous system lymphoma:Report of 3 cases with literature review

Objective：To investigate the etiology,pathogenesis,diagnosis and effective treatment of primary central nervous system lymphoma（PCNSL）.Methods：The clinical data were reviewed in 3 patients with PCNSL regarding the clinical manifestation,imaging characteristics,pathological features,therapeutic profile and treatment effect.Results：The 3 cases were pathologically confirmed as diffuse large B cell lymphoma（DLBCL）and undergone systemic chemotherapy combined with intrathecal intervention,with or without the whole brain radiotherapy,and the symptoms were significantly improved for the 3.Conclusion：Most PCNSLs are non-Hodgkin lymphoma（NHL）involved in B cells.Intrathecal chemotherapy combined with systemic chemotherapy may produce favorable but mild adverse effects for PCNSL,suggesting that this is the one of the approach to the treatment of the disease.Some new drugs and treatment programs available seem effective,yet further evaluation is necessary.
