200例选择性脊神经后根切断术式改良疗效观察

为寻求一种对脑瘫双下肢痉挛治疗创伤小，且症状改善较彻底的手术方式。方法：依据腰骶部脊 神经后根应用解剖学研究结果，改良了传统的选择性脊神经后根切断术（ＳＰＲ）术式，即部分切断腰２，３，５和骶１脊 神经后根，保留腰２，４和骶１椎板及腰４脊神经后根的完整性，治疗脑瘫双下肢痉挛２００例。结果：改良ＳＰＲ术式 创伤小，脊柱稳固，运动改善早而快等，优于非改良ＳＰＲ手术切除腰２，４和骶１椎板及部分切断腰４脊神经后根的 疗效。结论：改良ＳＰＲ术式可行。

The curative effect of modified SPR in 200 cases

Aim:To seek a surgical mathod which can be used to treat spasm of the lower limbs due to cerebral palsy with small in jury and rather thoroughly improved symptoms.Methods:Based on the research achievement of applied anatomy of rhizotomy of the lumbar and sacral nerves,the lumbar nerves 2,3,5and the posterior root of the spinal nerve of sacral 1 wrer partly cut,and the lumbar nerve 2,4 and the laminae of sacrum 1 and the posterior root of lumbar nerve 4 were preserved .Results:The intact treatment of 200 cases of lower extremity spasm due to cerebral palsy showed the characteristics of small injury,stable spinal cord,and early and quick improvement of movement .The result is superior to that of the 100 cases of the controlgroup ,in which lumbar nerves 2 and 4 and sacrum 1 laminae were resected ,and the posterior root of the spinal nerve of lumbar nerve 4 were partly cut in the operation.Conclusion:The modified operation if successful.
