脊髓髓内血管母细胞瘤1例报告与文献复习

目的总结脊髓髓内血管母细胞瘤的诊治经验。方法回顾性分析1例脊髓髓内血管母细胞瘤病人的临床资料和493例文献报告病例的相关资料。结果行超选择性栓塞后获肿瘤全切除。随访1年,神经系统功能明显改善。结论脊髓血管DSA可定位、定性诊断脊髓髓内血管母细胞瘤,并能发现多发病灶,对制订治疗方案有益。如应用得当,术前超选择性栓塞是低风险的,其可减少术中出血,缩短手术时间,减轻手术伤残,增加肿瘤全切除机会。脊髓髓内血管母细胞瘤的最佳治疗,是在最少出血状态下手术全切除肿瘤。

Intramedullary hemangioblastoma a case report and literature review

Objective To summarize the experiences in diagnosis an d treatment of intramedullary hemangioblastoma （IMHB）.Methods Clinical data were analyzed retrospectively in a patient with IMHB and 493 similar case reports in the literature.Results Total tumor removal was achieved after superselective embolization in this patient.During a follow-up period of 1 year,neurological function was obviously improved.Conclusions Spinal vessel DSA is able to make the topical and qualitative diagnosis of IMHB,and to reveal the multiple foci,which is helpful to develop the therapeutic strategy.If applied according to strict anatomical and technical guidelines,preoperative superselective embolization of hemangioblastoma is a low-risk procedure,and can decrease intraoperative bleeding,shorten the operative time,lower the surgical disability,and increase the chances for total tumor removal.Total removal of IMHB with minimal intraoperative bleeding is considered to be the best therapeutic option.
