摘    要:目的总结水环境神经内镜手术的操作技巧。方法回顾性分析316例水环境神经内镜手术病人的临床资料,归纳手术技巧。结果随访212例,平均随访时间2.1年。效果良好185例(87.3%),无效再治疗21例,恶化或放弃再治疗6例。29例发生并发症,其中硬膜下积液10例,颅内出血9例,颅内感染9例,头皮切口脑脊液漏5例,脑神经损伤4例,下丘脑损伤3例。7例发生永久并发症。死亡1例。结论水环境神经内镜技术处理非交通性脑积水、颅中窝蛛网膜囊肿、桥前蛛网膜囊肿、透明隔囊肿和脑室内囊肿效果很好,可作为首选治疗方法。

Abstract:Objective To summarize the technical skills of water-medium neuroendoscopic operations. Methods Data of 316 patients undergoing water-medium neuroendoscopic operation were analyzed retrospectively, and the surgical techniques were summarized. Results Two hundred and twelve patients were followed up with an average period of 2.1 years. One hundred and eighty five patients achieved good results (87.3%), 21 were retreated after failure in the first time, and 6 gave up retreatrnent due to disease aggravation. Twenty nine patients suffered complications after operation, 10 out of them subdural hygroma, 9 intracranial hemorrhage, 9 intracranial infection, 5 cerebral spinal fluid leakage from head incision, 4 cranial nerve injury and 3 hypothalamic injury. Seven patients had permanent complications, and 1 patient died. Conclusion Water-medium neuroendoscopic technique can yield a good therapeutic effect and can be used as the first-choice treatment for non-communicating hydrocephalus, middle cranial fossa arachnoid cyst, prepontine arachnoid cyst, septum pellucidum cyst and intracerebroventricular cyst.
