经静脉二次谐波声学造影诊断肝血管瘤的研究

摘    要:目的:探讨经静脉二次谐波声学造影诊断肝血管瘤的应用价值。方法:31例患者接受造影检查,造影剂经左前臂静脉注射,间歇成像由心电图R波触发。结果:注射造影剂后动脉相观察,17例强回声型、4例弱回声型和5例混合声型肝血管瘤表现“晚显影、晚消退”特点,瘤体边缘强化,呈“环状”强回声,中国未强化呈弱回声。另2例强回声型和3例混合回声型血管瘤表现“早显影、早消退”特点,瘤体呈均匀增强回声团块。门静脉相,瘤体均无明显再次增强效果,延迟成像,瘤体均呈回声增强团块,与周围正常肝组织相近。结论:经静脉二次谐波声学造影有助于肝血管瘤的准确诊断。

Abstract:Objective:To evaluate the diagnosis of hepatic hemangioma by using intravenous second harmonic imaging(ISHI).Methods:Thirty one cases were examined with ISHI.The contrast agents were administrated through left forearm veins.The intermittent imaging was triggered by R wave of electrocardiogram(ECG).Results:After the contrast agent was admitted,the characters of late echo enhancement and late disappearance were showed in the arterial phase in 26 hemangiomas including 17 with strong echo,4 hypoechoes and 5 mixed echo mode,and "circle like" echo enhancements were seen in the margins of the hemangioma,with hypoechoes appearing in the center of the hemangioma.The rest 2 strong echo and 3 mixed echo hemangiomas,displayed the characters of early echo enhancement and early disappearance,and the homogeneous enhancement were seen in hemangioma.In portal phase,no enhancement was observed in the hemangioma.In delayed phase,the echoes of hemangiomas were homogeneously enhanced,and the degree of enhancement were almost equal to the normal liver parenchyma.Conclusions:ISHI can make diagnosing hepatic hemangioma to a more accurate extent.
