胸腔镜肺叶楔形切除术治疗早期周围型肺癌的临床疗效

目的探讨胸腔镜下肺叶楔形切除术治疗早期周围型肺癌的临床效果。方法选择该院2007年1月—2011年8月住院的早期周围型肺癌患者24例,采用胸腔镜下肺叶楔形切除术治疗,肿瘤直径≤4cm,左肺上叶4例、下叶5例;右肺上叶5例、中叶2例、下叶8例。结果所有患者手术均顺利完成,肿瘤均完整切除,无破裂,无手术死亡。病理诊断结果为鳞状细胞癌9例,腺癌13例,细支气管肺泡癌2例。术后7例出现胸闷、气短、肺部哮鸣音,无呼吸衰竭等其他严重并发症发生,术后随访6～12个月,无肿瘤转移及复发。结论早期周围型肺癌经胸腔镜下肺叶楔形切除术治疗安全、可靠,具有患者创伤小、恢复快、痛苦轻、近期疗效确切的优点,值得临床推广应用。

Clinical Efficacy of thoracoscopic Wedge Resection in the Treatment of Early Peripheral lung Cancer

Objective To explore the clinical effects of thoracoscopic wedge resection in the treatment of early peripheral lung cancer.Methods 24 cases of early peripheral lung cancer patients hospitalized from Jan.2007 to Aug.2011 were selected.All patients were given thoracoscopic wedge resection with tumor diameter ≤ 4cm.Tumors located on the left upper lobe in four cases,the left lower lobe in five cases,the right upper lobe in five cases,the middle lobe in two cases and the right lower lobe in eight cases.Results All operations were successfully completed.Tumors were all resected totally without rupture.The result of pathological diagnosis showed 9 cases,of squamous cell carcinoma,13 cases of adenocarcinoma and 2 cases of bronchioloalveolar carcinoma.After surgery,seven cases presented dyspnea,shortness of breath and pulmonary wheeze No respiratory failure and other serious complications occurred.There were no tumor metastasis and recurrence patients after followed up for 6 to 12 months.Conclusion Thoracoscopic lobectomy wedge resection is safe and reliable in the treatment of early peripheral lung cancer with less trauma,light pain and quicker recovery,which is worthy of clinical application.
