心理干预对脑卒中后抑郁患者康复依从性及康复效果的影响

目的 观察心理干预对脑卒中后抑郁患者康复依从性及康复效果的影响.方法 将80例脑卒中后抑郁患者随机分为心理干预治疗组40例和常规对照组40例,两组患者均予早期康复和脑卒中常规药物治疗,治疗组实施心理干预.采用汉密尔顿抑郁量表（H AMD）、改良爱丁堡-斯堪的那维亚量表（MESSS）和日常生活活动量表（ADL）,分别对治疗组和对照组患者在治疗前和治疗8周后进行评定,并进行康复依从性评定.结果 两组治疗后HAMD、MESSS、ADL评分及康复依从性与同组治疗前比较,差异均有统计学意义（P＜0.05或P＜0.01）;治疗组治疗后HAMD评分及临床总有效率、MESSS、ADL评分及康复依从性与对照组比较,差异均有统计学意义（均P＜0.05或P＜0.01）.结论 对脑卒中后抑郁患者,在早期康复治疗的同时,实施心理干预,可以显著改善抑郁症状,提高患者康复依从性,促进脑卒中后神经功能的恢复和日常生活活动能力,提高患者的生活质量.

Influences of psychological intervention on rehabilitation compliance and effect of patients with post-stroke depression

Objective To observe the influences of psychological intervention on rehabilitation compliance and effect of patients with post-stroke depression.Methods 80 patients with post-stroke depression were randomly divided into treatment group and control group of 40 cases each.The two groups were treated with early rehabilitation and routine drug.The treatment group was implemented with psychological intervention.Both of the two groups were assessed by Hamilton Depression Scale（HAMD）,Modified Edinburgh Scandinavian Stroke Scale（MESSS） and activities of daily living scale（ADL） before and 8 weeks after treatment.The rehabilitation compliance was evaluated.Results After treatment,the two groups had marked changes in HAMD,MESSS,ADL scores and rehabilitation compliance （P〈0.05,P〈0.01）.The differences of HAMD scores and total effect rate,MESSS,ADL scores and rehabilitation compliance between the treatment group and the control group were statistically significant （P〈0.05,P〈0.01）.Conclusion For patients with post-stroke depression,the early rehabilitation treatment and the implementation of psychological intervention can obviously improve the symptoms of depression,enhance rehabilitation compliance and improve the life quality.
