重复经颅磁刺激及其磁共振功能成像的研究进展

摘    要:重复经颅磁刺激利用电磁感应原理非侵入性地刺激大脑皮层,从而改变刺激部位及与刺激部位存在解剖连接部位的皮层兴奋性。动物实验已证明重复经颅磁刺激可对皮层兴奋性产生持久性效果,并有效改变突触连接性。因其无创、无辐射且操作简便,重复经颅磁刺激被广泛应用于神经病学、精神病学、康复医学及认知领域,成为一种最具潜力的临床治疗方法。本文对重复经颅磁刺激从动物实验到中风、抑郁、帕金森等临床研究进展情况进行评述。并对未来的研究方向进行探讨,提供实时交互式经颅磁刺激功能磁共振成像在中风病人中的初步研究情况。

Abstract:Based on the principle of electromagnetic induction,repetitive transcranial magnetic stimulation (rTMS) can non-invasively give a brain stimulation and change the excitability at the site of stimulation as well as at distant anatomically connected sites.Animal experiments have confirmed the outlasting effects of rTMS to the excitability of the brain cortex and the efficiency to the changes in synaptic connections.Characterized by non-invasive,atraumatic and simple operation,rTMS has become one potential therapeutic intervention in a wide range of neurological,psychiatric,rehabilitation and cognition conditions.The present article will review the progress of rTMS from animal experiments to clinical application which include stroke,depression,Parkinson's disease and so on.Besides,we will discuss the future research directions and give some pilot study results of concurrent rTMS-fMRI on Chinese stroke patients.
