超声显像对肾脏恶性肿瘤的诊断与鉴别诊断价值

目的探讨超声检查对肾脏恶性肿瘤定位、定性及鉴别诊断价值,以提高超声诊断肾脏恶性肿瘤的水平。方法回顾性分析经手术或穿刺活检病理证实为肾脏恶性肿瘤的318例患者术前超声诊断准确率,并分析超声对肾脏恶性肿瘤漏误诊的原因。结果术前超声诊断与手术和病理对照,超声定位诊断符合率96.5%(307/318例),定性诊断符合率89.9%(286/318例),漏诊率1.9%(6/318例),误诊率8.2%(26/318例)。结论超声显像可动态观察肾脏肿瘤的形态及内部结构变化与特点,诊断准确率高,能及时为临床选择治疗方案提供重要参考依据;但也需注意个别患者的特殊表现与鉴别,尽量避免、减少漏误诊病例。

Diagnostic and differential value of ultrasonography on malignant renal tumors

Objective To discuss the sonographic appearance of malignant renal tumors,and assess the value of ultrasonography in the topical,qualitative and differential diagnosis.Methods Retrospective analysis of the preoperative sonographic appearance was performed on 318 cases.The diagnosis of ultrasound was confirmed by pathology.Results Compared to the operative findings and the pathologic diagnosis,the accordance rate of topical diagnosis of ultrasound was 96.5％ （307/318）,and that of qualitation was 89.9％ （286/318）.The rate of missed diagnosis was 1.9％ （6/318）,and misdiagnosis was 8.2％（26/318）.Conclusion Ultrasonography is a powerful method for real time visualization to malignant renal tumors and can provide abundant and important information for decision making of the clinical therapeutic regimen.
