NIX-1与BAP在类风湿性关节炎诊断中的应用价值

目的 将骨代谢指标Ⅰ型胶原氨基末端肽（NTX-Ⅰ）、骨源性碱性磷酸酶（BAP）与传统使用的经典检测方法类风湿因子（RF）、C反应蛋白（CRP）、红细胞沉降率（ESR）比较,分析对不同病期的RA患者的敏感性和特异性,为RA早期诊断、确诊、判断发展及疗效评定提供可靠依据.方法 健康对照组50例,RA缓解组50例,RA活动组50例.均分离血清,测定血中CCP、RF、CRP、ESR、NTX-Ⅰ、BAP.结果 RA活动组与对照组、缓解组比较,CCP、RF、ESR、CRP、NIX-Ⅰ、BAP,差异有统计学意义（P＜0.05）.RA缓解组与RA对照组比较,CCP、RF、NIX-Ⅰ水平变化明显,差异有统计学意义（P＜0.05）,而ESR、CRP、BAP水平变化差异无统计学意义（P＞0.05）.结论 NTX-Ⅰ在RA患者的诊断以及预测病情进展、观察临床治疗效果方面具有重要的意义,可与CCP／RF联合运用于RA的诊断中,可明显提高诊断率.

The value of index of traditional rheumatoid arthritis,stress response and bone metabolism in the diagnosis of rheumatoid arthritis

Objective To analyze the sensitivity and specificity of RA patients for early diagnosis of RA through comparing NTX-Ⅰ and bone alkaline phosphatase （BAP） with rheumatoid factor （RF）,C-reactive protein （CRP）,Cyclic citrullinated peptide（CCP） and erythrocyte sedimentation rate （ESR） Methods 50 patients with rheumatoid arthritis （RA） were included as RA group.50 patients in remission were included as remittent group.50 healthy patients were included as control group.CCP,RF,CRP,ESR,NTX-Ⅰ and BAP were measured in peripheral blood.Results CCP,RF,ESR,CRP,NIX-1 and BAP were up-regulated in RA group compared with that of RA group（P〈0.05）.BAP had no statistical significance in remittent group compared with RA group （P〉0.05）.CCP,RF,ESR,CRP and NIX-Ⅰwere up-regulated in remittent group compared with that of RA group（P〈0.05）.CRP,ESR and BAP had no statistical significance in remittent group compared with that of RA group （P〉0.05）.Conclusion NTX-Ⅰ has significant value in early diagnosisof RA patients,predicting disease progression and observing treatment effect.NTX-1 jointed CCP／RF can improve the diagnosis of rheumatoid arthritis.
