彩超检测非酒精性脂肪肝患者血流动力学改变对评估脂肪变程度的应用价值

摘    要:目的探讨彩超检测非酒精性脂肪肝患者血流动力学改变对评估脂肪变程度的应用价值。方法对2011年4月~2013年1月收治的非酒精性脂肪肝病患者104例进行彩色多普勒超声检测血流动力学改变,根据患者肝脏脂肪浸润程度不同将104例患者分为轻(46例)、中(33例)、重度(25例)组,另选取同期接受健康体检的30例健康人群作为对照组,对比四组患者的血流动力学指标。结果四组患者之间的各波形构成比例均存在显著差异(P<0.05)。4组之间的门静脉最大血流速度(PPVV)两两比较均存在显著差异(P<0.05)、平均速度(MPVV)轻度组和中度组两两比较无显著差异(P>0.05),其他各组之间两两比较存在显著差异(P<0.05);肝动脉阻力指数(HARI)各组之间两两比较均存在显著差异(P<0.05)。结论彩色多普勒超声检测非酒精性脂肪肝患者血流动力学改变可以准确评估肝脏脂肪变的程度,且具有无创、经济及简单易操作等优点,具有良好的临床应用价值。

Abstract:Objective To investigate the ultrasound testing in patients with non-alcoholic fatty liver hemodynamics in assessing the degree of steatosis.Methods 104 patients with non-alcoholic fatty liver disease were divided into mild (46 cases),moderate (33 cases) and severe groups (25 cases) according different degree and controlled with 30 healthy people.The hemodynamics was observed.Results The constituent ratio of waves and the maximum velocity of portal vein of the 4 groups was different (P<0.05).The average velocity of portal vein of mild group and moderate group was not different (P>0.05),but the other groups were different (P<0.05).The hepatic artery resistance index of the groups was different(P<0.05).Conclusion Color Doppler ultrasound in patients with nonalcoholic fatty liver hemodynamics can accurately assess the degree of hepatic steatosis and possess without invasive,economics,and easy to operate.
