分化型甲状腺癌的手术治疗分析

目的总结分析分化型甲状腺癌的诊断和治疗方法。方法回顾性分析2005年1月至2010年3月期间我院收治的80例分化型甲状腺癌患者的临床资料及随访结果。结果本组80例患者均顺利完成手术,术中及住院期间无1例死亡;术后病理证实乳头状癌52例(占65.0%),滤泡状癌28例(占35.0%);有淋巴结转移者18例(占22.5%);术后发生并发症者一共有5例(占6.25%);随访2～7年,术后存活率为97.5%。结论对于分化型甲状腺癌我们应当在术前做出明确的诊断,常规切除肿块或(和)可疑淋巴结送术中冰冻病理快速检查,以便在手术时能够根据患者肿瘤病理组织大小、浸润范围及颈部淋巴是否结转移等情况来决定手术方式,并在术后给予甲状腺素内分泌进行治疗。

The Analysis of Surgical Treatment of Differentiated Thyroid Cancer.

Objective To summary and analyze diagnose and therapy of differentiation type thyroid carcinoma. Methods Retrospectively analyze clinical data and follow-up visit of patients with differentiation type thyroid carcinoma. Results 80 patients were successfully suffered operation without death case. Pathology researches indicated that these were 50 cases of papillary carcinoma （65.0%）, 25 cases of cryptae carcinoma （35.0%）; 18 cases of lymphonode transfusion （22.5%）. 5 patients presented complication （6.25%）. survival rate was 97.5% after operation during 2-7 follow-up visit. Conclusion Definite diagnose before operation is important to patients with differentiation type thyroid carcinoma. And common cutting tumor or/and doubtful lymphonode for rapid pathological examination is benefit to operating selection according to size of tumor pathological tissue, infiltration scope, and whether transfusion of cervical lymph node and et al. Also patients should suffer therapy of thyroxine endocrine.
