脱细胞异种真皮移植覆盖技术在大范围皮肤缺损患者中的应用价值

目的 探究脱细胞异种真皮移植覆盖技术在大范围皮肤缺损患者中的应用价值。方法 选取2013年7月~2015年1月间笔者医院收治的87例大范围皮肤缺损患者为研究对象，随机将其分为研究组44例，对照组43例。研究组患者实施自体微型皮移植后脱细胞异种真皮移植覆盖术治疗，对照组患者实施自体微型皮移植后脱细胞异体真皮移植覆盖术治疗。观察两组患者手术指标，近期愈合情况，术后6个月使用温哥华瘢痕评分量表对患者远期疗效进行评价。结果 两组患者手术时间、切痂面积、术中输液量、RBC输注量、操作人数相比差异均无统计学意义（P>0.05）；研究组患者创面移植结果和对照组等级比较差异无统计学意义（P>0.05）；研究组患者温哥华瘢痕评分色泽评分、血管分布、厚度评分、柔软度评分和对照组相比差异均无统计学意义（P>0.05）。结论 脱细胞异种真皮移植覆盖技术对大范围皮肤缺损患者有良好的的治疗效果，可代替异体皮源，具有较高的临床应用价值。

Application Values of Cover Technique of Acellular Dermal Acellular Dermal Graft in Patients with Large Area Skin Defect

Objective To explore the application values of cover technique of acellular dermal acellular dermal graft in patients with large area skin defect. Methods Eighty seven cases of patients with large area skin defect in our hospital from July 2013 to January 2015 were selected as the research subjects,who were randomly devided into study group with 44 cases and control group with 43 cases.The study group were given surgical treatment of acellular dermal acellular dermal graft after autologous skin grafting,and the control group were given transplantation of acellular allogeneic dermis after autologous skin grafting.The operation indexes and recent healing of the two groups were observed,and the conditions of wound healing after 6 months of operation were evaluated with the Vancouver scar scale. Results There was no significant difference in operation time, the area of the tangential excision, the volume of transfusion, the amount of RBC infusion, and the number of operations between the two groups (P>0.05).There was no significant difference in the level comparison of wound transplantation outcomes between the study group and the control group (P>0.05).There was no significant difference in the scores of color, blood vessel distribution,thickness,softness of the Vancouver scar scale between the study group and the control group (P>0.05). Conclusion The cover technique of acellular dermal acellular dermal graft has significant effects on patients with large area skin defect,which can save the skin source,with high clinical application values.
