结核感染T细胞斑点试验(T-SPOT.TB)在不明原因发热成人患者疾病诊断中的应用价值研究

目的 探讨结核感染T细胞斑点试验（T-SPOT.TB）在成人不明原因发热（fever of unknown origin，FUO）中的诊断价值。方法 回顾性分析笔者医院近5年来以不明原因发热收治入院的成人患者共172例，统计性别、年龄、血常规、T-SPOT.TB和最终诊断。结果 T-SPOT.TB诊断试验在FUO患者中诊断的敏感度为82.14%，特异性为83.33%，准确性为83.14%，同时血常规指标中血红蛋白含量在结核病患者和其他疾病患者之间有显著差异，其曲线下面积为0.63，临界值为111.5g/L。结论 T-SPOT.TB诊断试验有助于FUO患者的结核病诊断。尤其T-SPOT.TB阴性患者可进一步排除结核病。

Analysis of T-SPOT.TB in the Diagnosis of Adult Fever of Unknown Origin

Objective To investigate the diagnostic value of T-SPOT.TB test among adults with fever of unknown origin (FUO). Methods A total of 172 patients with FUO as the admission diagnosis were retrospectively enrolled for evaluating the blood routine examination, T-SPOT.TB and the final diagnosis. Results The sensitivity of T-SPOT. TB diagnostic test in patients with FUO was 82.14%, the specificity was 83.33%, accuracy was 83.14%. At the same time, there was a significant difference of the hemoglobin content between patients with tuberculosis and other diseases, the area under the curve was 0.635, cut-off value was 111.5g/L. Conclusion T-SPOT. TB diagnosis test was useful to diagnosis tuberculosis in patients with FUO. Especially, T-SPOT.TB negative can further eliminate active TB infection in FUO.
