经鼻塞持续正压给氧治疗婴幼儿肺炎合并呼吸衰竭疗效观察

目的观察经鼻塞持续正压给氧治疗婴幼儿肺炎合并呼吸衰竭的疗效。方法 98例患儿随机分为治疗组50例和对照组48例,2组均给予综合性治疗,包括抗感染、解痉、雾化、拍背、吸痰等。对照组采用面罩吸氧,治疗组采用经鼻塞持续正压给氧治疗。观察2组症状、体征缓解时间和临床疗效。结果治疗组紫绀消失时间、呼吸困难缓解时间、肺部啰音消失时间、平均住院天数均明显短于对照组,差异有统计学意义(P<0.01)。治疗组总有效率为92.0%明显高于对照组的58.3%,差异有统计学意义(P<0.01)。结论经鼻塞持续正压给氧治疗可迅速纠正低氧血症,减少气管插管机械通气,同时减少对患儿刺激,患儿耐受性好,家长能接受,为治疗婴幼儿肺炎合并呼吸衰竭的有效方法,值得临床推广应用。

Effect observation of Nasal Continuous Positive Airway Pressure in the treatment of infantile pneumonia complicated with respiratory failure

Objective To observe the efficacy of Nasal Continuous Positive Airway Pressure（NCPAP） in the treatment of infantile pneumonia complicated with respiratory failure.Methods 98 cases were randomly divided into treatment group of 50 and control group of 45.Both groups were given comprehensive treatment,including antiinfective,antispasmodic,atomize inhale,pat back and suck sputum,etc.The control group was given oxygen by mask;And the observation group was treated by NCPAP.The symptoms,time of signs remission,and clinical efficacy of 2 groups were observed.Results The time of cyanosis disappearance,time of difficulty breathing remission,time of pulmonary rales disappearance and average hospitalization time of treatment group were shorter than those of control group,and the differences were statistically significant（P0.01）.The total efficacy of the treatment group was 92.0% higher than 58.3% of the control group,and the difference was statistically significant（P0.01）.Conclusion NCPAP can rapidly correct hyoxemia,reduce tracheal re-cannula mechanical ventilation,reduce stimulation to child patients,well tolerated and be acceptable to patriarch,which is the effective treatment for infantile pneumonia complicated with respiratory failure,so it is worth clinical popularizing and application.
