摘    要:目的研究静脉药物配置中心(pharmacy intravenous admixture services,PIVAS)对临床医疗质量的提升作用。方法对齐鲁医院PIVAS开业前后临床输液反应的发生率、不合理医嘱发生率、每床每周护理时间、患者平均用药量、平均每袋金额等指标进行比较分析。结果PIVAS建立后,临床因药物配置引起的输液反应发生率为0,不合理医嘱的发生率由建立前的33%下降至0.2%,每周每床增加护理时间约4.1h,患者每日平均用药量从3.1袋下降到2.2袋,每袋金额从112.87元下降到109.39元,患者每日药费平均降幅达31.22%。结论PIVAS可以保证用药安全、增加护理时间、降低患者药费,是代表医院药学发展趋势、提升临床医疗质量的全新服务模式。

Abstract:Objective To explore the effect of pharmacy intravenous admixture services(PIVAS) on improving clinical medical quality.Methods The incidence of clinical infusion reaction,the incidence of unreasonable medical order,the nursing time for each bed each week,the daily mean drug consumption,and the average cost for each medicine bag were and compared analyzed before and after the PIVAS implementation.Results After PIVAS implementation,the incidence of clinical infusion reaction dropped to 0;the incidence of unreasonable medical order dropped from 33% to 0.2%;the nursing time for each bed increased by 4.1 hours per week;the patient daily mean drug consumption dropped from 3.1 bags to 2.2 bags;and the mean cost per drug bag decreased from 112.87 RMB to 109.39 RMB.Conclusion PIVAS can guarantee clinical safety,increase nursing care time,and cut down medicine expense.PIVAS is a new medical service mode which represents the tendency of hospital pharmacy development and can promote clinical care quality.
