健康体检人群血尿酸水平与代谢综合征的相关性研究

摘    要:目的探讨健康体检人群血尿酸(BUA)水平与代谢综合征(MS)的相关性。方法选择2012年1月~2013年1月在解放军总医院体检中心进行健康体检、资料完者且均签署知情同意者6422名作为研究对象,比较不同年龄段男性、女性体检人群BUA水平及高尿酸血症情况;比较不同BUA水平体检者各代谢指标[空腹血糖(FBG)、总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、体重指数(BMI)]情况;分析BUA水平与各生化指标的相关性。结果入选的3666名男性体检者BUA水平为(348.3±87.2)μmol/L,发生高尿酸血症826例,占22.53%,以30~39岁构成比最高,达25.30%。入选的2756名女性体检者BUA水平为(278.3±71.7)μmol/L,其中发生高尿酸血症352例,占12.78%,以50~59岁构成比最高,达31.97%。高尿酸血症组体检者FBG、TC、TG、LDL-C、BMI水平及MS发生率分别为(6.38±1.32)mmol/L、(6.12±1.07)mmol/L、(2.21±0.74)mmol/L、(0.81±0.32)mmol/L、(26.5±7.1)kg/m2、26.2%(309/1178),明显高于血尿酸正常组,而高尿酸血症组体检者HDL-C[(1.21±0.16)mmol/L]明显低于血尿酸正常组(P〈0.05)。BUA与TC、TG、LDL-C、BMI呈明显正相关(r=0.019、0.281、0.157、0.245,P〈0.01);而BUA与HDL-C呈明显负相关(r=-0.189,P〈0.01),但BUA与FBG无明显相关性(P〉0.05)。结论 BUA水平与MS关系密切,它们相互作用、相互影响,应进一步加强和重视体检人群BUA水平的检测,以及其与MS的基础和临床干预研究,从而为MS的预防及治疗提供新的途径。

Abstract:Objective To explore the correlation between blood uric acid(BUA) level and metabolic syndrome(MS) in healthy population receiving physical examination. Methods 6422 people with complete health record and informed consent that received physical examination in the Physical Examination Center of General Hospital of the People's Liberation Army from January 2012 to January 2013 were selected as research objects. The study of BUA level and hyperuricemia in male and female of different age groups were compared; metabolic index(FBG,TC,TG,LDL-C,HDL-C,BMI,MS) in populations of different BUA levels were compared; the correlation between BUA level and biochemical index were analyzed. Results Among 3666 men that received physical examination,the average BUA level was(348.3 ±87.2) μmol/L. 826(22.53%) had hyperuricemia,and the highest hyperuricemia rate was observed in the group aged 30-39(25.30%). Among 2756 women that received physical examination,the average BUA level was(278.3±71.7) μmol/L. 352(12.78%) had hyperuricemia,and the highest hyperuricemia rate was observed in the group aged 50-59(31.97%). The average level of FBG,TC,TG,LDL-C,BMI levels and the incidence rate of MS was(6.38±1.32) mmol/L,(6.12±1.07) mmol/L,(2.21±0.74) mmol/L,(0.81±0.32) mmol/L,(26.5±7.1) kg/m2,26.2%(309/1178) respectively in the hyperuricemia group were significantly higher than the normal BUA group(P 〈0.05). The hyperuricemia group had significantly lower HDL-C level [(1.21±0.16) mmol/L] than the normal BUA group(P 0.05). BUA had significantly positive correlation with TC,TG,LDL-C and BMI(r = 0.019,0.281,0.157,0.245,P 0.01),and significantly negative correlation with HDL-C(r =-0.189,P0.01). However,BUA had no correlation with FBG(P〈 0.05). Conclusion There is close correlation between BUA level and MS with interactions on each other. We should enhance and attach great importance to BUA detection in people receiving physical examination,basic and clinical intervention researches on the correlation between BUA and MS,to provide new approaches for prevention and control of MS.
