特种蛋白检测在老年肺炎中的临床意义

摘    要:目的 :评价前白蛋白 (PA)、转铁蛋白 (TRF)和C -反应蛋白 (CRP)在区分老年急性肺炎病原体中的作用 ,并探讨其临床应用价值。方法 :采用免疫散射比浊法 ,测定老年肺炎患者治疗前后的血清PA、TRF和CRP水平。结果 :细菌感染组患者治疗前CRP水平明显高于健康对照组 ,而PA和TRF水平明显低于健康对照组 ,两者差异有显著性 (P<0 .0 0 1) ;病毒感染组与对照组比较 ,PA、TRF和CRP差异无显著性 (P >0 .0 5 ) ;细菌性肺炎患者治疗前CRP水平明显高于治疗后 ,而PA、TRF水平明显低于治疗后 ,两者差异有显著性 (P <0 .0 0 1) ;血清PA、TRF和CRP阳性率高于白细胞计数、红细胞沉降率 (ESR)和细菌培养的阳性率 ,两者差异有显著性 (P <0 .0 0 1)。结论 :血清PA、TRF和CRP的定量检测可用于老年肺炎病原体的鉴别及疗效的观察 ,其价值明显高于白细胞计数、ESR和细菌培养 ,是反映老年肺炎感染的敏感且可靠的指标 ,值得临床推广应用

Abstract:Objective To evaluate the function of PA, TRF and CRP to define the acute pneumonia pathogen and explore its clinical significance in aged people. Methods To check concentration of PA, TRF and CRP in serum sample from aged people between before and after the treatment with immune scattered nephelometry. Results ①CRP increased obviously in bacteria infection group than the control group, but PA and TRF decreased obviously. There are significant (P<0.001) between this two group;There is little difference (P>0.05) between virus infection group and the control group;CRP in bacteria infection group before treatment were significantly higher in those after treatment (P<0.001), but PA, RF were lower. There are significant difference (P<0.001) between the two group. Conclusion The change of PA, TRF and CRP level can be used to identify the acute pueumonia pathogen in aged people and observe the effect. Their value is higher than WBC counting ESR and bacteria culture. They are sensitive and dependable index that reflects acute pneumonia of aged people.
