顽固性心动过速的消蚀术治疗（58例报告）

消蚀术治疗５８例不同类型的心动过速，总成功率为９３．１％。其中４０例房室折返性心动过速有４５条房室旁道．３６例和４１条旁道成功地被消蚀。１５例房室结双径路致房室结折返性心动过速（消蚀慢径路）和３例室性心动过速（２例位于左室前壁，１例在左后隔）全部获得成功消蚀。无严重并发症。１例遗留有右束支阻滞。随访未见复发病例。根据单极电图的ＰＱＳ波形易取得成功和减少放电次数。为防止希氏束受损，在房室结处消蚀时必须密切监视心电图和消蚀导管的放置部位。

Treatment of Refractory Tachycardia with Radiofrequency Catheter Ablation(Report of 58 Cases)

Fifty six patients with different tachycardia were treated by radiofrequency catheter ablation.The total success rate was 92.86%.Among them 40 patients with atroventricular(AV)reciprocated tachycardia had 45 AV accessory pathways(AP9),36 of 40 patients(90%)and 41 of 45 APs(91.11%)were successfully abolished.In 13 patients with AV node reentrant tachycardia when ablation on slow pathway,that is all successful 3 patient with sustain ventricular tachycardia(VT);two patients ablation on the left-front wall of the left ventriculus.Other one was on the left-post septum,all the ablation succeeded.No any seriouscomplication occured,but a right bundle branch block in one patient. No relapse case was found at present.According to the PQS shape of the unipolar electrogram on ablation catbeter,the ablation will be more easily success and reduce the delivered current times.In order to prevent His bundle damage,the site of ablation catheter and the feature of its electrogram must be close monitor when the AV node was ablated.
